3. Two (2) copies of NBI Clearance
4. Three (3) copies of PRC Board Rating (Certified True Copy by the issuing agency)
5. Three (3) copies of Valid PRC License (Certified True Copy by the issuing agency)
6. Three (3) copies of PRC Certificate of Good Standing (Certified True Copy by the issuing agency)
7. Two (2) copies of Medical Certificate
8. Laboratory results of the following:
   
   a. Blood Tests (CBC)
   b. Urinalysis
   c. Chest Xray
   d. Drug Tests
   e. Neuro Psychiatric Examination
    
9. Three (3) copies Statement of Assets Liabilities & Net Worth (SALN)
10. Diploma and Transcript of Record (Certified True Copy by the issuing school/university)
11. Philippine Statistics Authority (PSA) Birth Certificate
12. Philippine Statistics Authority (PSA) Marriage Certificate
13. Clearance Original or CTC from previous employer (if any)
14. Clearance of Employment from previous employer (if any)
