4	3 copies of PRC Board Rating (Certified True Copy by the issuing agency)
5	3 copies of Valid PRC License (Certified True Copy by the issuing agency)
6	3 copies of PRC Certificate of Good Standing (Certified True Copy by the issuing agency)
7	2 copies of Medical Certificate
8	LAB RESULTS of the following: a. Blood Tests (CBC) b. Urinalysis c. Chest Xray d. Drug Tests e. Neuro Psychiatric Examination
9	3 copies of Statement of Assests, Liabilities & Net Worth (SALN)
10	Diploma and Transcript of Record (Certified True Copy by the issuing school/university)
